Medical Records Request

HIPAA regulations allows you to request copies of your personal health records.

You may download an authorization for release [English] [PDF] or authorization for release [Spanish] [PDF] form and return to us via e-mail or fax, 515-239-2049. The release form must be filled out in its entirety, and signed in order to be processed. When picking up your records, a valid picture ID is also required.

Request for Vital Records (birth certificate)?

Birth certificate certified copy requests must be made through Story County Vital Records. A $15 fee is required to search for a record and includes one certified copy. Each additional copy of the same record is an additional $15
