Fill out our online registration form to let us know you want to deliver your baby at Mary Greeley Medical Center.

How to Fill Out this Form

You should:

• Complete this form a few weeks before your due date.
• Allow at least 48 hours for us to process your form submission. Our staff checks form submissions Monday through Friday, 8 a.m. to 4:30 p.m. If there are less than 48 hours before your due date, a nurse will call you.
• Have your insurance cards nearby.
• Fill out the entire form. Fields marked with an asterisk (*) are required. The form will not submit if any required fields are left blank. If a question for a required field does not apply to your situation, write "NA" or "Not Applicable."
